Authorisation for Background Check

Have you ever been charged with a criminal offence in Mauritius or abroad? Do you have any offences outstanding?

Do you suffer from any health problems or disabilities that could prevent you from carrying out your work safely?

I hereby give my consent to Eagle Protect Ltd (including its members of staff) to run any and all background checks necessary to verify all the information I have submitted on this form.

This includes, but not limited to, questions that will be asked about my educational background, attendance, personal history, character, personality, disciplinary information and reasons for separation from my former employer/s.

All information acquired will expressly be used in determining my suitability and acceptability for employment with Eagle Protect Ltd. I, also hereby release Eagle Protect Ltd from all liability for damages or claims – which may arise or result from any reference information gathered pursuant to this authorisation.
